Why Two Inhales? The Physiological Sigh Explained

The double inhale is the single most distinctive part of cyclic sighing. Here's what it's doing, in plain terms.
A Reflex the Body Already Has
Every so often — roughly every few minutes, even at rest — the body performs a spontaneous physiological sigh: a deep breath followed by a second, smaller breath, then a longer exhale. One proposed function is re-inflating small collapsed air sacs (alveoli) in the lungs, which can gradually deflate during normal shallow breathing.
What Changes When You Do It on Purpose
Cyclic sighing takes that existing reflex and repeats it deliberately, at a steady pace, instead of waiting for it to happen spontaneously. The second inhale — the "top-off" — tops up lung volume just past what the first inhale alone would reach.
Why the Exhale Matters So Much
The extended exhale is arguably the more important half of the pattern. A long, slow exhale is the mechanism researchers most often point to when discussing how paced breathing may influence physiological arousal — it's the opposite pattern from rapid, shallow breathing, which tends to do the reverse.
A Few Form Notes
- The first inhale should be full but comfortable — not a maximal breath.
- The second inhale should be short — a small top-off, not another deep breath.
- The exhale should be roughly twice the length of both inhales combined — slow and controlled, not rushed.
If you feel lightheaded, the first inhale is usually too forceful, or you're moving through rounds too quickly. Ease off and slow down, or stop and breathe naturally if the sensation doesn't pass.
